Advances in Cosmetic Dentistry


For most people, cosmetic dentistry used to mean getting braces on their teeth. There were other things, such as having caps put on to whiten the teeth or correct different types of flaws, but those kinds of things were mostly for the rich or famous.

These days, however, there have been so advances in this field, and it has become more common in mainstream society. With that, prices for many procedures have gone down. Though dentists still put braces on people's teeth and still whiten them with caps, they do so now in much more innovative ways.

Old Methods
If you want to go really far back, you'll see methods of cosmetic dentistry consisting of dentures carved out of wood, and teeth whitening done by barbers using nitric acid. Those were fairly ancient, and obviously flawed, methods and procedures.

As little as two or three decades ago, however, dentists were using porcelain materials to cap teeth and fill in gaps, and they've used metal braces to straighten teeth for many decades. Teeth whitening by barbers using enamel-destroying acids gave way to fluoride lozenges that also tended to brighten the smile, although later it was found that over-use of fluoride actually discolored the teeth.

New Methods
Today, cosmetic dentistry has made many strides forward. Although people do still get metal braces to straighten their teeth, the practice is rapidly becoming less common. These days, for themselves and for their children, people are opting for the new, clear, plastic aligners that you wear in stages, with each stage straightening your teeth just a little more until the desired result is finally reached.

These aligners are completely removable, which means that you can take them off to eat or brush your teeth, and the treatment is usually only needed for about a year or less. That is a significant improvement over metal braces, which some people had to wear for up to 3 or 4 years. Though dentists still use porcelain to cap teeth and fill gaps, they do so in a new way that bonds everything better.

Dentures, though still regularly used (and made out of acrylic, not wood), are starting to give way more and more to dental implants, which are basically prosthetic teeth that eventually bond naturally to the gums. Whitening procedures are much safer for the teeth, and there are a variety of them to choose from.

The Future
What should we look forward to in the field of cosmetic dentistry? Basically, just more improvements in the methods that are being utilized now. More than likely there will be advancements beyond that as well. 3-D Digital imaging of teeth, which is being used now in some cases, will probably play an increasing role in developing cosmetic procedures and prosthetics.

The good news is, cosmetic dentistry is available to a wider range of people today, including teeth-whitening treatments that can be bought over the counter at your local pharmacy. No doubt this will continue to be a positive trend.

Should Teens Be Getting Plastic Surgery?


Teenage women (those under 18 years old) account for about 2-3% of all patients presenting to plastic surgeons offices to discuss cosmetic surgery. Personally, I have seen an increase in both inquiries as well as consultations by teenage patients. Some of the more common reasons young women present for cosmetic surgery is for breast augmentation, liposuction, and rhinoplasty. This is controversial. The issues involved include; maturity, decision making ability, safety and legal aspects.

Maturity and Judgment

There are several good reasons that age restrictions have been placed on all sorts of things that require maturity and judgement such as purchasing a gun, driving and drinking alcohol. The frontal lobe of the brain does not fully develop until the 20's to 30's.  This portion of the brain controls decision making, impulsiveness and understanding consequences. There have been studies to suggest that 20-30% of high school students consider suicide. This is also a period with a high incidence of experimentation with illicit drugs. Clearly, adolescence is a period of time where judgement can be challenged. This is not to say that adults always make proper decisions for themselves, adolescents are simply at a distinct biologic disadvantage. The reasons a teen may want plastic surgery can be questionable. "Fitting in", or wanting to look like a specific air-brushed celebrity aren't good reasons to pursue plastic surgery. This is a time of self-discovery, not self-alteration.

Safety

In general, the risks of cosmetic surgeries are similar among adults and adolescents. However, there are instances when operating before full development can effect the cosmetic outcomes. For example, breast surgery before full breast development or rhinoplasty before the nose has completely developed.

The Law

There is no specific legislation in the United States regarding teenage cosmetic surgery. As with any medical procedure, parental consent is required for patients under 18 years of age. In Australia, there has been legislature to make it difficult for teenagers to have cosmetic surgery including a "cooling off" period between consultation and surgery. There have been discussions of similar laws in the U.S.   For breast augmentation, The American Society of Plastic Surgeons and the FDA have both taken a position. Women under 18 are not approved to have saline implants and those under 22 are not approved for silicone implants. There are life-long risks associated with breast implants such as capsular contracture and deflation that become greater the longer that implants have been in place.

Reconstructive vs. Cosmetic plastic surgery

In my practice, all patients presenting for procedures that are purely cosmetic must be old enough to sign their own consent form (18 years of age) and must be suitable candidates for the planned procedure with realistic expectations. The difficulty is that some of these patients present with problems that are both cosmetic and reconstructive in nature. For example


  • Children with prominent ears resulting in self-consciousness 

  • Girls with extremely asymmetric breast development

  • Girls with extremely large breast development limiting physical activities

  • Boys with male breast development

  • Facial scarring from a previous injury

  • A nose deformity causing significant breathing problems

There is clearly a continuum from purely cosmetic to purely reconstructive surgery. It takes the experience and judgement of a good plastic surgeon to make the decision of whether an operation is indicated while the patient is still a minor or should be required to wait until they are 18 in these tough cases. If you've got an interest in science, medicine as well as art, you may think about becoming a plastic surgeon. Since it is very expensive, check that you will have the financial resources necessary to fund your schooling.
 
Complete the work of pre-med. Once you are done with college, you will need to finish a pre-med program. In this period, you are enrolled in general medical classes and can't declare your specialty in cosmetic surgery yet. Courses will include the following: biochemistry, anatomy, pediatrics, gynecology, etc. With the help of this, you can work in all fields of medicine. After you reach third year pre-med studies, you may declare cosmetic surgery as your special field. Check the Plastic Surgery Board of America website to confirm your interest in this career. If you keep working in the medical field you will have more chance to be admitted to a plastic surgery residency.
 
To become a plastic surgeon resident, there is one of two options you can choose. When you are done with your pre-med, the next step will be a completion of a residency. There are two choices for this. The first is 3 years of general surgical residency, then 3 years of plastic surgery residency, under the independent model. However, if you choose the integrated model, you can focus on necessary surgical training for your entire 6-7 year residency.

Although the bizarre moniker "twilight surgery" has eerie ramifications of Rod Serling's popular Twilight Zone television program of yesteryear, it actually is becoming an increasingly popular way to undergo some cosmetic procedures - without the administration of general anesthesia.

It might sound scary or perhaps even a little bit shocking, but in reality the benefits of twilight surgery are numerous. In fact, if you think you can handle the idea of a plastic surgeon cutting off part of your face, manipulating the bones in your nose or shaving off some ear cartilage while you're wide awake, this could be the surgery for you.

Why Twilight Surgery?

Twilight surgery, which originated in the United States but is now becoming popular virtually worldwide, is carried out while patients are awake, although they are put under heavy sedation before going under the knife. In some cases patients wake up having no recollection of the procedure; in others, they remember every nip, cut and tuck.

How it works is simple. Instead of administering a general anesthetic, the surgeon uses a local one along with a sedative, one that makes the patient feel comfortably drowsy and relaxed. This allows him or her to talk during the surgery, in some cases, and actually move and be responsive to the surgeon when required.

Also known as twilight sleep, conscious sedation or monitored sedation, twilight surgery is not for the squeamish, even though both the local anaesthesia and sedative are administered by a qualified anesthesiologist. The latter may include nitrous oxide (laughing gas) or intravenous medications, and for patients who do not want to run the risk of general anesthesia, the benefits are obvious.

General anesthesia can be dangerous for certain people, such as heavy smokers, those who are dangerously overweight or those with certain medical conditions. Often patients worry that they will be put under and never wake up. When it comes to elective surgery, which plastic surgery more often than not is, twilight surgery can take away a considerable risk.

Local anesthesia as opposed to a general is beneficial for other reasons as well. For procedures such as a facelift, twilight surgery will allow patients to smile, laugh and frown during the course of the operation, so that the surgeon can check the movement of facial or wrinkles and, ultimately, achieve better results. With liposuction, the surgeon can ask the patient to stand up so her or she can access the result of their handiwork once gravity has taken its toll. This would not be possible under heavier sedation.

Types of Twilight Surgery

As far as plastic surgery is concerned, the consensus is mixed. Most surgeons would agree that facelifts, tummy tucks, eye lifts, ear pinning and nose jobs, also known as a rhinoplasty, can be carried out advantageously as twilight surgery. Others say that invasive procedures such as a breast enlargement operation can also have good results, while others believe it's unsafe.

The decision, ultimately, is up to you and your surgeon - but make sure you only use one accredited by the American Society of Plastic Surgeons, or its equivalent, for best results. And as twilight surgery is relatively new, only use a surgeon who has done the procedure before many, many times before. Remember, you only live once - so ensure you do the best to take care of your health while you can, and don't stint on money when it comes to something like an operation.

Disadvantages of Twilight Surgery:

In some case, twilight surgery is there to alleviate anxiety if a patient has had a traumatic experience while under general anesthesia. So if the thought of being awake during your operation causes you evenmore undue stress, perhaps you should opt for a general anesthetic.

Other disadvantages include...

* In some cases, the patient could move or flinch during the operation, not a good idea when a surgeon is wielding a large knife...
* Patient could suffer a bad experience during the operation and later feel traumatized
* Often patients can feel a minor pulling and/or tugging during surgery, which some may find unpleasant

Advantages of Twilight Surgery:

* The advantages are numerous, although remember that twilight surgery is not suitable for every type of cosmetic procedure...
* Patient can respond to surgeon and move face or other body parts during the operation
* Quicker recovery period than surgery performed under general anesthesia
* No complications or difficulties associated with general anesthesia
* Shorter hospital stay, in most cases, as opposed to surgery done under general anesthesia
* No breathing tube required during surgery or after

Many patients say they have less bruising and bleeding afterwards, as well as no post-operative nausea, which is a common after-effect of general anesthesia

Twilight sleep is commonly used during dental surgery with few complications, although some people worry (unduly) that they will reveal some embarrassing secret to their dentist while they are under, or will do something they will regret. Remember that everyone has a different reaction to specific medications, and that a true professional will not even notice if you act a bit more brazen than usual - they may even enjoy it!

The Cosmetic Dentistry Procedures Dental Insurance Generally Excludes


The level of dental insurance cover, like all other insurance, tends to vary from policy to policy but as a general rule cosmetic dentistry procedures, for the sake of appearance, are not covered by dental insurance.

One main indicator as to what won't be covered by dental insurance is whether or not the dental care you are seeking is for the sake of appearance or for the ongoing care of your teeth.

o Take for example tooth whitening. Tooth whitening is a cosmetic procedure with its only impact being on the colour of your teeth. Standard dental insurance policies will not cover cosmetic procedures such as tooth whitening

o Dental implants are likely to be an exclusion on a standard dental insurance policy with such dental work being for appearance and comfort rather than the care of your teeth. Unless you have opted for an insurance (which obviously comes at a price) that includes certain cosmetic procedures the high cost of tooth implants could be coming straight out of your own pocket.

o The fixing of porcelain veneers is another procedure that is unlikely to be covered by dental insurance, again, a cosmetic procedure purely for the sake of appearance. It might be possible to claim the cost of defective cosmetic dental work from your dental insurance but not the cost of the original work.

o Basic braces are often covered by dental insurance but if you want something more sophisticated such as invisible braces then you will generally have to cover the cost yourself.

o Dental insurance might or might not cover the cost of tooth bonding with it basically coming down to the reason the work is being done.

o Dental insurance is likely to cover the cost of amalgam fillings but if you want the more visually appealing white fillings then expect to pay.

o If you need a crown and there is no other option then your dental insurance provider is likely to insist on the cheaper option of porcelain fused to a metal crown. It will do the job can look quite fake. If you want a natural looking porcelain crown then you'll need to cover the additional cost.

This is just a short summary of likely dental insurance exclusions. It's a rule of thumb and not a given so make sure you never pay out for dental insurance until you know what you are going to get for your money. Compare policies so that you get best value for money and make sure it is clear as to what costs you will have to cover yourself.

Be warned. One story I heard just the other day was that someone lost and damaged a significant number of teeth when he was pushed to the ground hitting his mouth full on. His dental insurance covered less than half of the $15,000 dental bill!

Wound Care After Surgery - How to Heal Faster After Having Plastic Surgery


Wound care after surgery can take a toll on a person! To ensure the best results whether you are choosing plastic surgery to alter your looks, or having reconstruction done because of breast cancer, a good outcome comes from knowing how to take care of yourself before and after your procedure. The better you prepare - the better your outcome will be!

Keep in mind that about 1 in 100 wounds will become infected after surgery no matter what you or your doctor does. We will look at ways to make sure you are not that one.

Your wound care after surgery actually starts weeks ahead of time in getting your body ready.

To prepare your body for your surgery start a few weeks ahead of time in getting good nutrition, properly hydrated and plenty of rest and good exercise. Think of this as an athletic event you are getting ready for.

Being in good physical shape helps your body heal faster because good circulation is vital to getting blood, oxygen and nutrients to the surgical area.

Your skin is the largest organ of the body - and the thinnest. Only 1/10 of an inch separates you from all sorts of toxins. The skin is also highly permeable - what goes on your skin can get in your blood stream. You need to be good to your skin and prepare your skin for the upcoming incision or damage that is going to happen. When the skin gets what it needs; vitamins, minerals, proteins and nutrients, it can do what it does best - heal itself.

You need to make wise eating choices, and drink plenty of water. Your diet should include healthy proteins which are building blocks for cells, vitamin C and zinc which are known to promote wound healing. Lots of fresh green vegetables will also help speed up the recovery process. Eliminating processed sugars and carbonated beverages also helps the body mend quicker.

Being in good physical shape will help speed up your recovery as well. This does not necessarily mean go join a gym and go crazy - simply get your body moving. You will do yourself a favor to take a 30 to 40 minute walk each day just to get your blood moving.

Staying properly hydrated helps a body heal better as well. There is a simple way to know how much water is the right amount for you to be drinking daily. You need half your body weight in ounces. For example; if you weigh 150 pounds you should drink half that in ounces: 75 ounces of water daily. That will help flush toxins out of your system and help your skin heal where the incision is going to be.

Lack of sleep takes a toll on your immune system. Try to get a good 7 - 8 hours or more of sleep per night in the weeks leading up to your surgery. Naps are a good thing when needed as well.

Do I really need to say how harmful smoking is to your health and healing process? And please - no alcoholic beverages for a few weeks before at best, and at the least for one week prior to your surgery. Then be kind to your body and refrain for a few weeks after as well.

Afterwards, your healing is helped by continuing to drink plenty of water and getting your needed sleep. Get up and about as soon as possible - the body mends better when the blood is flowing and circulating. Talk with your doctor about what you physically can and cannot do.

If the skin around the wound becomes red, swollen and painful you may have an infection. Call your doctor.

In healthy patients following the wound care after surgery, most wounds heal within two weeks. If you have health problems such as diabetes or a weakened immune system, or are on certain drugs, such as steroids or chemotherapy drugs your healing will probably take longer.

Wounds on the face usually take one to two weeks to heal. Wounds on the arms, trunk and upper legs take about three weeks. Wounds on the lower legs, especially in people over forty, can take up to 3 months to heal.

The word 'laser' conjures up very strong emotions in many of my patients: ranging from pure elation to extreme aversion. This comes from a lack of understanding of the very concept of laser, not to mention the fact that many have strong misconceptions about lasers.

Quite a few think they are a cure-all, while others tell stories of someone who was sidelined for weeks to nurse scarring from a laser blast.

What is LASER?

The truth is, Lasers are not at all that magical. They are, simply, machines which emit light.

The difference is that unlike conventional light sources, lasers emit intensely bright light of a fixed wavelength, which is in phase. (Think of a scenario where 3 children are on swings in a playground. If all of them are moving in tandem, they are in phase. If they way they move bears no relation to each other, they are out of phase).

By using different materials, the wavelength of the light emitted can be controlled. You have heard terms like Ruby Laser, Nd:Yag Laser, etc. These refer to the laser medium which is housed in the laser machine. They are the ones which determine the wavelength of the emitted light.

Now why is the wavelength important?

Different lasers target different skin component

Our skin has different components and each of these components absorb light in different ways with different wavelengths. By using different lasers and the appropriate settings, different structures in the skin can be targeted and selectively destroyed. One can target the melanin (pigmented component), the haemoglobin (in blood vessels), the hair follicles, or the water in the skin with different lasers.

Now we're going to really make them shine. Put a small amount of clear coat on a second lint free cloth. Rub it in smooth circular patterns over the entire wheel. Add clearer coat to get the look and shine you are after. You can now add a thin layer of spray wax if you want even more shine. Be sure to wear gloves the entire time to prevent your hands from becoming dry from the chemicals, and read all labels and warnings on the cleaning products. The first documented record of any kind of plastic surgery taking place was two thousand years before Christ in India. It was during the course of the first world war that many new techniques were pioneered. A New Zealand surgeon named Harold Gilles was instrumental in helping many young men who had badly disfigured by their injuries. His work was carried on during world war two by his cousin Archibald McIndoe.

McIndoe set up a burns unit that would become famous throughout the world for young airmen who had suffered terrible burns during the fighting. The members of this very select group were called the Guinea Pigs, and they had their own club called the Guinea Pig club. McIndoe helped many of these brave men to recover from their injuries with his pioneering surgeries.

The work of this burns unit still continues today in the McIndoe burns unit in the Queen Victoria hospital in the town of East Grinstead in southern England. The survivors of the Guinea Pig club still meet today. At the end of world war two it had nearly seven hundred members, and there are still two hundred remaining members who get together regularly.
