Patients coming in before surgery may have a common waiting room, but patients returning after procedures should have a separate waiting area or entrance to ensure that they can maintain privacy during their postoperative period. If there is not a completely separate area, the physician may arrange his/her schedule to ensure that postoperative patients are scheduled in such a way as to not be seen by other patients. Ask about the use of photographs

Plastic Surgery Laws - What is the Minimum Age to Have Breast Augmentation?


When it comes to the breast augmentation procedure, there are a few federal regulations that anyone needs to be aware of. For starters, even though a minor can go in for other forms of plastic surgery, unless it is a reconstructive procedure that minor cannot get a breast augmentation.

In 2000, the federal government passed a new regulation stipulating that no person under the age of 18 may have a breast augmentation procedure except for some rare occasions in which that breast augmentation is truly a reconstructive procedure. A few years later in 2006 another regulation was passed that further amended the age 18 stipulations for breast augmentations.

This new regulation that was passed in 2006 stipulated that no person under the age of 22 was allowed the option of silicone filled breast implants. Basically a person who is going in for a breast augmentation and is between the ages of 18 and 22 is only allowed the saline solution filled breast implants.

The reasons cited for this new stipulation regarding the silicone filled implants was due to the fact that a woman's breast have been know to still be in the process of forming well passed the age of 18 in into their 20's. This was also due to the belief that a young adult is still not mature enough mentally to make a sound decision regarding the procedure. There are also no known risks of implant rupture of a saline filled implant where as the silicone filled implants are still under investigation.

Even upon reaching the age of 22, it is important to always discuss the risks associated with young adult breast augmentations with your doctor as there may be some cases in which having the procedure formed by be more detrimental to the patient's health.

If you are between the ages of 18 and 22, you should most likely wait to be sure that your breasts have completed their growing process before contemplating undergoing a breast augmentation. Should you be one of the many minors who are under the age of 18 and contemplating a breast augmentation, then chances are you are only thinking about it to be like someone on TV. It is in this reasoning that the federal government has disallowed persons under the age of 18 from undergoing a breast augmentation.

Botox: Its Latest, and Most Exciting Cosmetic Uses


Undoubtedly, the fastest growing non-surgical cosmetic procedure is Botox injections, with over 1.3 million administered last year alone. Type-A exotoxin, or Botox Cosmetic®, marketed by Allergan, Inc., Irvine, CA, is produced, perhaps surprisingly, by the bacterium Clostridium botulinum, the cause of botulism.

While skin researchers have demonstrated that wrinkles in the aging face are clearly related to the accumulated effects of excessive sunexposure, smoking, volume loss, and gravity, facial expressions and animation, known as dynamic wrinkling, also play a major role in the development of many types of neck and facial lines and furrows. By binding to the junction between nerves and muscle tissue, Botox effectively blocks the release of acetylcholine, the chemical responsible for normal muscular contractions, weakening the ability of certain facial muscles to induce fine lines, wrinkles, frowns and furrows through their repetitive use.

Botox has an excellent safety record in humans. Since the late-1970s, it has been used to treat muscle abnormalities associated with certain tic disorders, vocal cord problems and ocular abnormalities. And in the more than twenty-five years since its introduction and FDA-approval for these indications, it has consistently proven not only safe but reliable in millions of injections.

Cosmetic Uses of Botox

While other procedures, like laser resurfacing, dermabrasion and chemical peeling deal primarily with wrinkled skin at rest, Botox is considered the treatment of choice for movement-related wrinkles. Although initially used only to treat the so-called "scowl" (frown) lines between the eyes, Botox injections have been found remarkably effective for treating a wide range of facial and neck lines and furrows. Great improvement may be seen in the horizontal "worry" lines of the forehead and the "crow's feet" lines on the sides of the eyes, which in many cases may be eliminated entirely with treatment. Even more recently it has been used as a chemical (nonsurgical) browlift to restore the more youthful-appearing subtle upturning of the lateral eyebrows, to reduce the crepe paper-like crinkles under the eyes and creases around the chin, to counter the "marionette" line downturning at the corners of the mouth, to soften "smoker's" (lipstick bleeding) lines around the lips, and to reduce laugh lines. Even the so-called "bunny" lines on the sides of the nose may be eliminated by Botox. In the neck, Botox has been employed successfully for improving the appearance both of the vertical "chicken-neck" ("turkey gobbler") ropelike bands on the upper neck and the horizontal "necklace" lines encircling the lower neck. Even more recently, it has been successfully employed to lift the tips of drooping noses, a common problem with aging. Results of these treatments are usually seen between one and ten days after treatment and typically last for four to six months, although after several treatment periods more prolonged benefits may be seen.

Best Candidates for Treatment

While the "typical" Botox patient is usually a woman in her mid-forties to mid-sixties, many men and women older and younger have also been treated successfully. Individuals with neuromuscular disorders and pregnant women and nursing mothers, however, are not candidates for treatment.

What To Expect

Botox is a watery-looking substance, which is administered, for greater comfort, through a needle finer than most sewing needles. The actual number of injections and the amounts given vary with the areas being treated. Prices for treatment sessions also vary considerably, depending upon locale and the number of sites treated, and may range from $500 to $2000.

Certain medications and food supplements may increase the possibility of bruising, so you should discuss with your doctor stopping them prior to treatment. These include aspirin, non-steroidal antiinflammatory drugs (eg. Advil, Aleve), alcohol, vitamin E, ginseng and ginko biloba. Although injections tend to be somewhat uncomfortable, a simple ice pack applied beforehand generally suffices to reduce discomfort. It take about fifteen minutes to treat an entire face and neck.

Happily, complications of Botox treatments are few and short-lived, and Botox can truly be counted among the "lunchtime beauty fixes," meaning that in almost all cases, patients can immediately return to work or social activities. Minor complications include swelling, bruising and tenderness at the injection sites, which usually disappear in a few days; slight headache, which may last a few hours; and rarely, a slight lid drooping, which most often resolves in two weeks. There have been no reports of generalized illness from treatments with Botox. One treatment session is often sufficient, but If a touch up is needed, it is best done at least a month later to reduce the chance of inducing any resistance to the material.

To prevent Botox from spreading beyond the treatment sites, a few physicians advise their patients to remain upright for at least four hours immediately afterward and not to massage the treated areas. Most simply advise patients to repeatedly contract the treated muscles for up to hours hours afterward in order to work the material into the body of the muscles.

Some patients fear, unreasonably, that following Botox therapy they will have a "Kabuki-like" mask appearance and that they will also lose skin sensation. In reality, only the treated muscles will be relaxed leaving all else functioning perfectly intact, including sensation. In the overwhelming majority of cases, Botox patients are delighted with their "calmer," more relaxed, less wrinkled, less frowning look. And the future of these treatments looks bright as other forms are being studied and newer uses being tested.

So scientists have been looking at ways to improve on this technology and not only increase its efficacy in terms of skin rejuvenation, but also remove any associated discomfort and downtime; and now they think they've cracked it with the introduction of hydradermabarasion, the crystal free microdermabrasion.

What is Hydradermabrasion and What Can it Treat?

Hydradermabrasion, marketed by the innovative company behind the first technology, as the Hydrafacial(TM) treatment, is indeed similar in concept to its older cousin; however the primary difference is the lack of any crystals used to assist in the exfoliation process, and the use instead of a specially shaped spiral tip which exfoliates and removes impurities along with delivering pneumatically applied serums that cleanse, hydrate and provide antioxidant infusion during the treatment process.

The HydraFacial(TM) system was first introduced at the American Academy of Dermatology Annual Meeting in 2005. It won the industry's "Best of The Best Award" in 2006 and was nominated for the award again in 2007 and 2008. It became available in the UK in 2009.

Hydradermabrasion takes its name from the word 'hydrate' meaning to 'cause to take up moisture'. This ability to actively moisturise the skin during the treatment process sets it apart from all other skin resurfacing procedures available, such as microdermabrasion, chemical peeling and laser/IPL treatments.

According to the manufacturer the Hydrafacial(TM) treatment is clinically proven to improve the appearance of fine lines, wrinkles, congested and enlarged pores (e.g., blackheads), oily or acne-prone skin and hyperpigmentation (i.e. dark areas of the skin, such as brown age spots).

How Does it Work?

The special tip, which is the unique part of the Hydrafacial(TM) devices, is known as the Hydropeel(TM) Tip. It is passed over the surface of the skin in smooth, stroking movements, whilst proprietary serums are simultaneously directed onto it and then vacuumed away, along with loosened dead skin cells and other debris. All the waste is retained in a separate closed container to avoid any risk of contamination. Treatment is generally considered to be painless and a new treatment tip is used per patient.

The treatment works differently from traditional microdermabrasion due to the spiral shape of the tip, which has multiple abrasive edges, meaning that the skin is exfoliated several times as the tip is moved over it in a planing motion, whilst the serums applied soften the skin and sebum (skin oils) making it much easier to extract away impurities.

Treatment is said to be safe for most skin types and colours, including thinning or ageing skin, dry or oily skin and ethnic coloured skins; however those individuals who have been treated with isotretinoin for severe acne within the last six months, those with active skin infections, such as herpes simplex (cold sores), or with a history of problems associated with skin healing are not suitable candidates for treatment.

The proprietary serums within the Hydrafacial MD(TM) range, which are delivered direct to the skin's surface via the Hydropeel(TM) Tip, include activ-4(TM) which contains glucosamine and lactic acid, as well as active botanicals for cleansing and hydrating all skin types, beta-hd(TM) which contains salicyclic acid for cleansing and hydrating oily and acne prone skin and antiox-6(TM) an antioxidant treatment serum for all skin types containing hyaluronic acid, vitamins A and E and white tea extract designed to combat the damage created by free radicals within the environment, and deeply moisturise the skin.

The manufacturers of the Hydrafacial(TM) device have also teamed up with leading skincare company Skinmedica® to enable them to offer their TNS® (Tissue Nutrient Solution) Serum which contains NouriCel MD(TM), a proprietary mix of human growth factors, cytokines, soluble collagen, antioxidants and matrix proteins, discovered through more than a decade of wound-healing research. Growth factors and cytokines are proteins that the body produces for regular growth and repair of tissue. The proteins are believed to work in concert with each other to produce the desired effects of reducing the appearance of fine lines, wrinkles and sun damage. A home use kit of Skinmedica products is often recommended following the Hydrafacial(TM) TNS® Serum treatment to enhance and maintain the results.

According to Dr Bruce Freedman, who has recently published a study on the use of hydradermabrasion for non-ablative facial rejuvenation looking at the efficacy of applying topical antioxidants on their own or mechanically applying them to post-hydradermabraded skin: "the key step is the pneumatic application of the topical antioxidants; manual application is not as effective"; he said.

Additionally the Hydrafacial(TM) system offers a chemical peeling treatment in conjunction with the hydradermabrasion procedure. There are three Glysal(TM) peel strengths available, ranging from the Mild Glysal(TM) Prep which is suitable for most skin types as a pre-peel or preparation product, the Moderate Glysal(TM) Peel for oily, acne prone skin and to target fine lines and hyperpigmentation and the stronger Aggressive Glysal(TM) Peel.

Finally red and blue LED (Light Emitting Diode) handpieces can be applied to the Hydrafacial(TM) treated skin to aid skin rejuvenation and acne treatment respectively.

The energy delivered by red LED light has been shown to enhance cellular metabolism, accelerate the repair and renewal of damaged skin cells, as well as stimulate the production of collagen leading to improved skin texture and firmness. The Del Sol(TM) red light handpiece emits 615nm red light. Unlike more powerful laser and IPL treatments there is no pain associated with the delivery of the LED light to the skin.

The Del Sol(TM) blue light handpiece emits 420nm blue light. Propionibacterium Acnes or P.Acnes bacteria is known as the cause of acne and porphryin which is a compound generated by P.Acnes is photosensitive and has been proven to react to the wavelength of 405 - 440nm (blue) light which leads to the destruction of the bacteria. The combination of Hydrafacial(TM) as a pre-treatment to the blue LED light means that dead skin and debris is removed first so that the blue light is better targeted at the acne prone skin layers beneath.

Treatment sessions usually take approximately 30 minutes, with most people returning to work or normal activities immediately after treatment with only slight redness to the skin and a possible tight feeling which should resolve in less than 1 - 2 hours. Treatments using the peel products may prolong any redness however. The manufacturers report that many people see visible skin improvement after just one treatment with the results and skin hydration lasting between 5 and 7 days, and sometimes longer, making it an ideal one off treatment for a special occasion or a 'freshen-up'. However, a series of six to ten treatments are usually recommended for improving the appearance of fine lines, wrinkles, acne and oily skin or areas of hyperpigmentation, with ongoing maintenance treatments every 2 to 8 weeks. A treatment programme incorporating the various serums, growth factors, peel products and LED light will be customised on a per patient basis, depending on the individual skin requirements.

The average cost of a basic HydraFacial(TM) treatment is approximately £100, with advanced treatments including LED lights, peels or growth factor serums being in the range of £125+; generally discount packages are available for a course of treatments. As this treatment is currently so new there are only a handful of clinics currently offering it within the UK.

Clinical Results

A number of clinical trials into the use of hydradermabrasion and the associated application of antioxidant serums, both as a stand-alone treatment and in conjunction with other non-surgical skin rejuvenation procedures, such as Intense Pulsed Light (IPL), have been carried out by influential American physicians Dr. Patrick Bitter and Dr. Bruce Freedman.

A study by Dr. Freedman involved 20 volunteers being split into two groups, one group had six hydradermabrasion treatments using the mechanically applied antioxidant serum, spaced 7 - 10 days apart, and the other group simply had the serum applied manually, again for six treatments spaced 7 - 10 days apart. Results gathered two weeks after the final treatment showed that the group treated with hydradermabrasion demonstrated increased thickness in the skin layers, with the deposition of new collagen fibres and increased levels of antioxidants in the skin. Fine lines, pore size and hyperpigmentation were also decreased following treatment. The other group showed no change in skin structure, clinical attributes (such as wrinkling, hyperpigmentation etc.) or antioxidant levels. It was therefore concluded the Hydrafacial(TM) treatment effectively improved skin quality and increased the antioxidant levels present in the skin, whilst the lack of an abrasion element and a manual application of antioxidants did not result in detectable skin changes.

If we are to criticise this study in any way, it would be to note that a third group was not established who were treated with traditional microdermabrasion and topical antioxidant application, so as to establish the exact effect of the differing types of skin abrasion (wet versus dry) and the subsequent take up of the serums by the skin.

In a study by Dr. Bitter, presented at the ASLMS conference in 2009, he looked at how hydradermabrasion worked in conjunction with IPL on facial skin. Two groups of patients were treated with either a series of IPL treatments alone or a combination of IPL and hydradermabrasion. In the combination group, results showed that fine lines, texture, pore size and overall skin appearance were improved. This study showed that hydradermabrasion appeared to enhance the effects of the IPL, which was due to the antioxidant infusion which is part of the Hydrafacial(TM) treatment's uniqueness.

Other Options
Although the Hydrafacial(TM) systems are becoming the most widely known and heavily marketed devices within the hydradermabrasion arena. There are some other systems available which also use the concept of high pressure fluids to exfoliate or hydrate the skin as part of an abrasion treatment.

JetPeel(TM)-3 from Israeli company TavTech Ltd and distributed in the UK by Cross Medical Ltd is also a hydradermabrasion device, in that it uses a pneumatically applied liquid to exfoliate the skin; however the company manufacturing it have chosen a name to market it aligned with a peeling process rather than a microdermabrasion process. Some clinics offering treatment with this device are however choosing to use the term 'hydrodermabrasion' to emphasise the use of water, rather than the implied peeling products.

Each JetPeel(TM) treatment effectively "pressure washes" the skin using microdroplets of water (saline) and pressurised oxygen or air, delivered at supersonic speed - up to 200m/sec. When the jet stream of droplets strikes the skin the kinetic energy of the droplets causes an exfoliation effect. It is claimed that this gentle cleansing unclogs pores and removes any dead skin cells and debris that has built-up on the skin. Unlike other systems, this device doesn't use any kind of vacuum system to remove the resulting waste. The saline can be combined with other topical products containing vitamins and supplements such as hyaluronic acid designed to address different skin conditions, which are said to penetrate better in combination with the superficial exfoliation achieved and barometrically pressure environment caused by the pressurised gas upon the skin.

U.S. company Genesis Biosystems, a long standing producer of microdermabrasion machines, launched the Accelafuze(TM) in 2008; (distributed in the UK by Eden Aesthetics). The device combines traditional microdermabrasion using crystals followed by a rapid 'dermalinfusion' of various topical solutions such as a glycolic acid based cleanser, moisturising, lightening and peptide replacement serums and a lactic acid based peel with LED treatment options using red, blue and yellow light.

Although, like the Hydrafacial(TM) system the Accelafuze(TM) targets the mechanical application of topical products and the use of LED light, the use of traditional crystal based microdermabrasion means that this device cannot truly be classed as a hydradermabrasion option as it still relies on the use of dry crystals for the exfoliation element.

Summary

The popularity of microdermabrasion as a long term treatment programme for maintaining a healthy glow to the skin to reduce the appearance of fine lines, wrinkles and sun damage is well established, along with its versatility as a one-off, special occasion treatment when you want to look your best for a wedding etc.

With the introduction of hydradermabrasion many within the industry are seeing this as the next generation of minimally invasive skin resurfacing which moves on from traditional microdermabrasion by offering a painless treatment with less post treatment redness, along with the pneumatic application of antioxidants directly to the skin during treatment, rather than by topical application post-abrasion.

As with many new technologies, manufacturer claims tend to be somewhat embellished, and we must also offer a small note of caution on their safety and efficacy, as until long-term clinical evidence is gathered, little is known of any potential future complications.

How to Treat Adult Acne


Acne vulgaris (commonly called acne) is a common skin condition, caused by changes in the pilosebaceous units. These units are skin structures consisting of a hair follicle and a sebaceous gland. The changes occur as a result of increased sebum production via testosterone stimulation. Both men and women have varying amounts of testosterone. Acne is most common during adolescence, affecting more than 85% of teenagers, and frequently continues into adulthood. This type of acne affects the areas of skin with the largest amount of sebaceous follicles. These areas include the face, the upper part of the chest, and the back. Whenever acne becomes inflammatory it can damage the skin by destroying the collagen. For most people, acne diminishes over time and tends to disappear-or at the very least decrease-after one reaches one's early twenties. There is, however, no way to predict how long it will take to disappear entirely, and some individuals may continue to suffer well into their thirties, forties and beyond.

Patients may be surprised to learn that development of acne vulgaris in later years is actually quite uncommon. True acne vulgaris in an adult woman may be a feature of an underlying condition such as pregnancy and disorders such as polycystic ovary syndrome or the rare Cushing's syndrome. It is also known that menopause-associated acne occurs as production of the natural anti-acne ovarian hormone oestradiol fails at menopause. The lack of oestradiol also causes thinning hair, hot flashes, thin skin, wrinkles, vaginal dryness, and predisposes to osteopenia and osteoporosis as well as triggering acne (known as acne climacterica). So what is adult acne and why does it occur in later life? Why does it also seem to affect mostly women and occur around the mouth area where there are fewer pilosebaceous units? In fact why does it last for years and not respond to the normal treatments such as Benzoyl peroxide or Salicylic acid 2% (Acnesal) products. I would also ask why is it so common and bothersome for so many people?.

Is adult acne really acne?

I think these effects happen because what we call adult acne is probably a totally different disease. Several factors are known to be linked to acne, including the tendency for the condition to run in families and exposure to certain chemical compounds such as dioxins. Remember the thirties are the decades of rosacea and what we see is often a different disease called perioral dermatitis masquerading as adult acne. Many GPs also call this acne and treat it the wrong way. Stress, through increased output of hormones from the adrenal (stress) glands causes outbreak of the condition. While the connection between acne and stress has long been debated, scientific research indicates that "increased perioral dermatitis" is "significantly associated with increased stress levels.

So what exactly is perioral dermatitis?

Perioral dermatitis is a condition closely related to acne vulgaris that affect young women between the ages of twenty and forty five. Occasionally men or children are affected. Perioral refers to the area around the mouth, and dermatitis indicates redness of the skin. In addition to redness, there are usually small red bumps or pus bumps, and mild peeling. Sometimes the bumps are the most obvious feature, and the disease can look a lot like acne. The areas most affected are within the borders of the lines from the nose, to the sides of the lips, and the chin. There is frequent sparing of a small band of skin that borders the lips. The skin lesions can affect the area around the eyes. It is not uncommon, and has a tendency to recurrence in individuals who have had it once. This condition is often related to stress and become common in summer time as it acts like rosacea, becoming worse with sunlight exposure. Sometimes there is mild itching or burning.

How long does it last?

If not treated, perioral dermatitis may last for months to years. Even if treated, the condition may recur several times, but usually the disorder does not return after successful treatment.

What causes perioral dermatitis?

The cause of perioral dermatitis is unknown. We know it is a neurodermatis and hence related to stress. Some dermatologists believe it is actually a form of rosacea or sunlight-worsened seborrheic dermatitis. We know that strong corticosteroid creams applied to the face can cause perioral dermatitis. Once perioral dermatitis develops, corticosteroid creams seem to help, but the disorder reappears when treatment is stopped. In fact, perioral dermatitis usually comes back even worse than it was before use of steroid creams. Some types of makeup, moisturizers, and dental products may be partially responsible. There is also a suspicion that fluorinated toothpastes are related to an outbreak of this condition.

Can it be prevented?

There is no guaranteed way to prevent perioral dermatitis. Do not use strong prescription strength corticosteroid creams on the face. Your dermatologist may have suggestions about the use of moisturizers, cosmetics, and sunscreens, and may advise against using toothpaste with fluoride, tartar control ingredients, or cinnamon flavouring.

Are laboratory tests needed to diagnose the problem?

Most of the time, no tests are necessary. A dermatologist can usually make an accurate diagnosis by just examining the skin. Sometimes, scraping or a biopsy of the skin is done. Occasionally, blood tests are ordered to eliminate other conditions that can look similar.

How is this condition treated?

Dermatologists tend to use oral antibiotics, similar to the ones we use in Rosacea to treat the condition. This means a patient would require taking doxycycline or tetracycline for minimum of 3 months to prevent recurrence. For milder cases or pregnant women, topical antibiotic creams may be used. Occasionally, your dermatologist may recommend a specific corticosteroid cream, just for a short time to help your appearance while the antibiotics are working.

Is this similar to the treatment of acne?

Yes and no. I suppose systemic antibiotics are a mainstay in the treatment of ordinary acne vulgaris. Some of these antibiotics, such as Doxycycline (ByMycin) and Minocycline (Minocin) have anti-inflammatory properties and generally more effective than tetracycline. However, resistance is becoming more common and other antibiotics, including Trimethoprim (Septrim) are reportedly more helpful in acne than perioral dermatitis. Roaccutane (Isotretinoin) is a systemic retinoid that is highly effective in the treatment of severe acne vulgaris. It does this as it depresses sebum excretion by 70%, is anti-inflammatory, and even reduces the presence of acne bacteria. I do not tend to use it with perioral dermatitis as the basis of the condition is not sebum related. Roaccutane is a teratogenic and pregnancy must be avoided. A negative pregnancy test result is required prior to the initiation of therapy. A doctor will also check your cholesterol and liver tests monthly.

Are lasers of any value?

Lasers that use Photopneumatic™ technology such as the PPx and Isolaz has little use treating this condition as the underlying problem is not related to an increase in sebum However IPL (as used in Rosacea) appears to be of some benefit in controlling the condition.

What can be expected with treatment?

Most patients improve within two months of oral antibiotics. If corticosteroid creams were used for treatment, there may be a flare-up when the creams are stopped. If antibiotic treatment is stopped too early, however, the problem can come back.

Are there any other treatments?

There are many OTC products available for the treatment of acne, many of which are without any scientifically-proven effects. Generally speaking, successful treatments show little improvement within the first two weeks, instead taking a period of approximately three months to improve and start flattening out. Many treatments that promise big improvements within two weeks are likely to be largely disappointing. However, short bursts of cortisone can give very quick results to ordinary acne but are not recommended for this condition. .

Topical bactericidals

Topical bactericidal products containing benzoyl peroxide may be used in mild to moderate acne. The gel or cream containing benzoyl peroxide is rubbed, twice daily, into the pores over the affected region. Bar soaps or washes may also be used and vary from 2 to 10% in strength.

I normally do not recommend benzoyl peroxide to be used in this condition as it is a keratolytic (a chemical that dissolves the keratin plugging the pores) and the primary problem is not due to blocked pores. Other antibacterials with less keratolytic effects include triclosan, or chlorhexidine gluconate.

Topical antibiotics

These include ointments such as erythromycin, clindamycin or tetracycline. They act by killing the bacteria that are harboured in the follicles. While topical use of antibiotics is equally as effective in ordinary acne as oral use, I do not find them as effective in this condition. However, sometime I use Rozex and Metrogel (metronidazole) in much the same way I would treat a Rosacea patient.

Hormonal treatments

In females, ordinary acne can be improved with hormonal treatments. The common combined oestrogen/progestogen pill has some effect, but the antiandrogen, cyproterone in combination with an oestrogen (Dianette) is particularly effective at reducing androgenic hormone levels. Most patients with adult acne are too old to use this drug so it is not generally used

Topical retinoids

This group of medications are used to normalise the follicle cell lifecycle. They include brands such as tretinoin (brand name Retin-A), adapalene (brand name Differin), and tazarotene (brand name Tazorac). Like isotretinoin, they are related to vitamin A, but they are administered as topicals and generally have much milder side effects. They can, however, cause significant irritation of the skin and I never use them in this condition.

Phototherapy

It has long been known that short term improvement can be achieved with blue and red light. Recently, visible light has been successfully employed to treat acne (phototherapy) - in particular intense violet light (405-420 nm) generated by purpose-built fluorescent lighting, LEDs or lasers used twice weekly has been shown to reduce the number of acne lesions by two thirds. It is even more effective when applied daily. The mechanism appears to be that a porphyrin produced within P. acnes generates free radicals when irradiated by 420 nm and shorter wavelengths of light. These free radicals ultimately kill the bacteria.

She said she gets these from her father's side - "My dad's and aunt's grooves are so big they can hold a dime in the slots!" she told me.

For some people, the wrinkles, lines and sagging may result from the drying and damage that sun exposure does to our skin. And aging, bringing the loss of the collagen and elastin that support the delicate skin of the face, leaves it vulnerable to gravity. But whatever the cause, there are non-invasive cosmetic dermatology procedures available that can reverse these effects.
