After the bruising and swelling finally went down (less than a week) the difference was amazing. I could see better and I looked so much younger. I was absolutely delighted. It set me off.

I contacted the same agent I had seen before, and she showed me how my face would look after surgery. This time I opted for a full face lift. The operation took 4 hours and this was done under general anaesthetic. I must admit I suffered a lot of bruising - more than most and it took about a week to settle down but after fortnight I was transformed. I found chewing sugar free dental gum useful, it helped me exercise my jaw and kept things moving. I looked better, felt better and started to look after myself more. It gave me the incentive to start exercising again. People were complimenting me on how well I looked. The incision lines were in well hidden places in my hair line and behind my ears, so there was no way anyone could actually tell that I had undergone surgery.

I was told the eye surgery would last 7 years but it is 15 years now and they still look good. Just a very slight wrinkling on the left side, which is easily correct with makeup. A good balanced diet of fresh organic fruit, vegetables and olive oil add to a healthy skin, which will always help set you on a good road to quick recovery.

How One Book Influenced My Decision To Become A Plastic Surgeon


There were no doctors in my family. We had businessmen, lawyers, accountants, teachers - no doctors. I had not ever even considered being a doctor. While in high school, I developed an avid interest in psychology which I pursued on my own, as well as academically, continuing on well into my college years as well. For a while, most of my potential career choices seemed to be heading along those lines. But then somehow, magically, I got bitten by the "Medicine" bug and knew that this was what I wanted to do; what I needed to do, with my life. It's certainly not a big jump from a love of psychology to a love of biology, but medicine beckoned me with an enticing mix of a little bit of everything...and so much more. My goal of becoming a physician became the driving force of my days. College turned into medical school. Then, for what seemed like forever at one time, but which now seems merely a blink, I graduated from medical school and began my very traditional five year general surgical residency.

Why Surgery? I had grown to recognize that I was more attracted to the philosophies and styles of the surgical specialties. I felt that my personality, thought process and skills as a physician seemed more suited toward the "surgical" rather than the "medical" fields. Ideally, surgery would be overall more satisfying to me and as a result, for my patients, too. Even as a medical student, my surgical rotation was filled with many sleepless nights, 14 hour days, and more reading than any human being could handle. And I had never been happier in my entire life. Definitely, people skills and a psychology background could only be of benefit here. But at this point for me, more of a background benefit than a foreground one.

Years passed and so did hundreds of trauma surgeries, ruptured aortic aneurysms, bowel resections, cancer surgeries, Surgical ICU patients, adults, children, minor surgeries, major surgeries, outpatient, inpatient, simple, complex, happy outcomes, sad outcomes, family dramas and, unfortunately, even deaths.

I began to envision what type of surgeon I might become. I had seen much, learned much and experienced much. I knew I had so much more to learn. Maybe, even more about myself. I thought about families and patients and what they felt and what I felt when it came down to the point where a life was in the balance and sometimes, that life was lost. The length of a person's life could always seem to have been cruelly short, especially in the case of a child. But most people realize eventually that the kind of life lived, the way it is lived, and how that life and love is shared with others far exceeds in importance the number of years to that life. At the scrub sink one day, our division head of plastic surgery commented to me that Plastic Surgery "may not add years to a life, but it is supposed to add life to the years". It was then that I suddenly recalled a book I had read when I was eighteen, written by psychiatrist Dr. Herbert R. Lazarus. Dr. Lazarus described how upset his father became when he informed him about his decision to become a psychiatrist rather than a "real doctor". What a waste of all that time, money, training and schooling - his father was openly and deeply disappointed. Dr. Lazarus was hurt but had no answer which would satisfy his father, and many others critical of his decision, at the time. Years later, he would be able to explain that his decision was the result of realizing that he felt a different calling. That he felt that it was more important for him as a physician to help people, to truly help people, by helping them to live better, more fulfilling and more rewarding lives than to just help them live longer or disease/illness free. Even today, although better than years ago, Psychiatry and the importance of mental health and well-being is still not held in the regard it should be - by physicians or the public. Yet, no matter what we do as doctors, in the end everything comes down to quality of life - where does the determination of that reside? It is in our minds and in our hearts.

The words of Dr. Lazarus had been smoldering in me all those years, waiting to be heard and answered by my conscience. What did I really believe? What kind of doctor did I want to be? I knew in my heart that I believed what Dr. Lazarus had said. It became very clear to me that as a plastic surgeon, a substantial part of what I would do as a physician would be to try to help improve the quality and joy of someone's days. Not just their special days, but their everyday's. The days that all add up one day to the story that is a life. Hopefully, a life which is that much sweeter, that much more enjoyable, and that much more of a pleasure, because of something I was able to do for that person. I completed my general surgery residency, obtained my board certification, and then continued on in my training in a wonderful plastic surgery residency. Now, some 35 years later, I can honestly say that becoming a plastic surgeon was the fulfillment for me of becoming just the kind of a physician I hoped that I could one day be. If I'm very lucky, I will continue to earn and deserve to be given the incredible honor and responsibility to make the kind of a difference for someone that makes life just a little better. Intense Pulsed Light (IPL) Treatments Ideal for Removing Acne Scars?

While cutting-edge lasers have proven themselves effective at removing acne scars, the premium price for laser treatments may not appeal to everyone. Happily, another more economical option for correcting facial scarring includes intense pulsed light (IPL) treatments.

Intense pulse light treatments are typically more affordable than laser treatments for cosmetics corrections because the machines usually cost less than lasers. Moreover, unlike most lasers treatments- that are typically exclusively performed by physicians- estheticians can perform IPL facials, which brings the price down substantially.

Several medical journals have published reports on the effectiveness of IPL treatments for treating scars. For instance, a study in the Journal of Cosmetic Dermatology examined how well IPL improved raised, inflamed and small scars and concluded that this method did show definite improvements in scarred skin.

Similarly, the Journal of Cutaneous Laser Therapy featured a study that reviewed how well IPL treatments promote new collagen growth. New collagen growth is essential for filling acne scar indentations and smoothing damaged skin.

Five volunteers took part in the study. First, researchers took samples from the patient's skin in order to measure changes in the skin's collagen levels. Next, each of the participants received four IPL sessions. Once the volunteers completed a series of IPL treatments, examiners took another skin sample from each subject.

After reviewing the before and after skin samples, every patient showed evidence of new collagen growth.

Since IPL treatments can help correct acne scarring, the next question is how long will the skin retain the improvements delivered by this type of phototherapy. An investigation that appeared in Dermatologic Surgery and that spanned five years answered this question.

In the study, eighty patients of various skin tones received IPL treatments to combat the signs of aging. Investigators took photos of the participants and measured the patients self-perceived improvements in skin texture and hypopigmentation after the IPL treatments. The volunteers had received an average of three IPL treatments.

Four years after the treatments, 83% of the participants could still see an improvement in skin smoothness while 79% were witnessed an improvement in hyperpigmentation clearing.

While studies suggest that IPL treatments can prove ideal for acne scarring, there are several factors to consider before opting for an IPL treatment to correct scars. The first is, how many treatments will you need to get the results you desire. In most studies, patients only received three IPL sessions to correct skin imperfections and skin discolorations.

Before your IPL treatment, explain to your physician or esthetician how you expect your skin to look after the IPL treatments. This way, you will get realistic numbers for how many treatments you will need and what improvements you can expect to see with your acne scars after you complete the IPL sessions.

Hyperpigmentation in Cosmetic Laser Procedures


Hyperpigmentation means the abnormal darkening of skin, usually in a specific area of the body. This may be due to an injury, such as a burn, or from inflammation, such as an infection (e.g. a skin infection that was treated). Normal pigmentation is from melanin which is produced by melanocyte cells in the skin. Anytime there is an inflammatory stimulus, melanocytes might be triggered to produce extra melanin as part of the healing process. Increased melanin production by the melanocytes occurs in response to thermal changes and inflammation. It usually goes away but may last anywhere from a few weeks to many months. It is different from scarring. Scarring means that there is increased collagen production in the area and that the texture of the skin has changed. Hyperpigmentation causes no textural change of the skin, just a change in color.

Laser treatment of the skin can also occasionally lead to hyperpigmentation. Any laser transmits light into the skin. If there is heat generated or cells damaged, inflammation and thereby hyperpigmentation may be stimulated. The ideal in most cosmetic laser treatment is to target specific elements within the skin, such as hair or age spots, and avoid thermal damage to surrounding structures. This kind of targeted approach is usually successful in avoiding hyperpigmentation.

In published research, it has been shown that darker-skinned patients are more likely to be affected by hyperpigmentation when treated for laser hair removal. Darker skin types may have a 10 times risk versus the lightest skinned patients. In hair removal, some lasers have a lower risk of hyperpigmenation than others. The diode laser, for instance, has a much lower risk than the ruby. For this reason, the ruby is no longer commonly used for hair removal.

THe main treatment for hyperpigmenation has been hydroquinone. It varies in strengths from 2% to 4% or much higher. Hydroquinone works by blocking the tyrosinase enzyme which is important in the making of melanin. It is not recommended for long term use but may be very useful in the short term to prevent hyperpigmentation in patients prone to it who are receiving laser hair removal or other laser procedures. It may even be used as pre-treatment in darker-complexioned patients. There are many natural alternatives to hydroquinone, such as kojic acid, azelaic acid, and licorice extract. Their effectiveness in comparison to hydroquinone has not been well-tested.
