Plastic surgery is a wonderful option for those with deformities. The majority of people in the world are not born with congenital deformities. Cosmetic Surgery For The Nipples


Inverted Nipples: One or both sides may be affected and to varying degrees. If the inversion of the nipple has been of recent onset, it is very important that a thorough investigation as to the possibility of breast cancer be pursued. Breast cancers can be associated with nipple inversion. Most cases, however, are essentially a simple nipple tissue abnormality that was actually present since birth but only first became noticeable during breast development and puberty. These are simply aesthetically objectionable. Mild cases often respond quite well to simple maneuvers performed during a breast enhancement (augmentation with implants) and are a welcomed "side benefit" to the procedure. More severe cases require some rather sophisticated and tiny incisions which are made in and around the nipple but usually with good success and minimum required in the way of a recovery. Most patients have these issues addressed while having a cosmetic breast procedure - such as a breast augmentation with implants, breast lift or breast reduction - in the operating room. Otherwise, it is quite realistic to have the nipple inversion correction performed alone as an office procedure (usually with local anesthesia and mild sedation).

Inadequate Nipple Length or Poor Projection of the Nipples: This can actually be due to a disproportion in sizes between the nipple and areola rather than a true nipple deficiency. The areola diameter may need to be decreased to make a better match. True length problems can be often be corrected with a minor surgical procedure similar to that used to correct inverted nipples as described above. Sometimes a long-lasting, injectable filler (such as we use for the face) can help in the improvement.

Overly Long or Large Nipples: Again, the possibility that this is actually due to a disproportion between the nipple and the areola sizes must be determined first. The areola diameter may need to be increased. Cosmetic tattooing is the simplest way to do this. In more severe cases of small to absent areola tissue, skin grafts of darker pigmented skin can be used. Otherwise, a surgical reduction in the actual length of the nipple is a very straightforward and relatively simple procedure which can be performed in an office setting. Puffy or overly "fat" nipples can also be thinned down a bit by a similar technique.

Overly Large Areola Diameter: Areola diameter reductions are often performed in coordination with a breast reduction or breast lift procedure in the operating room. We want the already excessively large areola to have good proportion to the newly lifted, compacted and re-shaped breast. Occasionally, an areola reduction will be performed alone. The new, smaller diameter is planned and the intervening ring of tissue is removed with the outer "circle" edge tightened in to fit. The scars tend to blend within the natural circle of the areola circumference. The human eye and brain are wired to expect to see this circle-like line which automatically makes it less likely that a scar mimicking this line will be visible.

Irregular Areola Border: The same techniques which are used to decrease the size of the areola are modified to create a smoother, more circle-like shape to the border of the areola. The scars generally hide within the natural circle that characterizes the areola margin.

Nipple is Off Center within the Areola: Usually fixed as part of a breast reduction or breast lift as this would be much harder to surgically fix otherwise. Cosmetic tattooing to balance the areola out is a good non-surgical option. Skin grafting is a much more aggressive alternative and rarely done for this particular problem.

Too Light, or Inadequate Areola Pigmentation: The best option for this, hands down, is cosmetic tattooing.

Nipple/Areola Complex Too High on the Breast: This is usually best treated by a breast enhancement with implants because in most situations the displacement is an optical illusion created by poor breast volume and awkward positioning of the tissue on the chest wall. True high displacement of the nipple/areola complex on the breast/chest is a tough problem otherwise - all existing techniques to move the complex lower will most likely result in an obvious scar on the upper pole of the breast/chest.

Nipple/Areola Complex Too Low on the Breast: This is a very common problem, often associated with large and/or drooping breasts. During a breast lift or a breast reduction, the complex is lifted to its proper position, resized proportionately and properly centered on the breast mounds. The nipple/areola complexes are placed so that they are in mirror image symmetry to the size, shape and position of the each other as much as possible. The scars hide within the circular edges of the areolas.

Nipple/Areola Complex Not Centered on the Breast: Many women have nipple/areola complexes which seem to be out toward the sides of the breasts. Bringing them inward so that the complexes are closer to the midline of each breast makes for a much more desirable look. Most effective solutions to this problem are as a part of a breast lift or breast reduction procedure as described above. More minor procedures which are modifications of some of the steps in a lift or a reduction can be performed for less severe cases or where the breasts are otherwise acceptable and not in any need of reshaping, resizing or lifting. When the complexes appear to be too close together (i.e. "cross eyed") a well-done breast augmentation will often result in a more centered and more pleasing look to their positions.

Overly Prominent or Numerous, Highly Visible Bumps within the Areola: These are known as "Montgomery Glands" and although perfectly normal, they are sometimes aesthetically offensive if too prominent or too numerous; they are very edgy, irregular and "bumpy". Simple excision works well - they do not typically recur.

Prominent Nipple/Areola Complex Hair Growth: Electrolysis is probably a better choice for this than would be laser hair removal. There are usually only a few hairs to treat and electrolysis is usually less expensive, more reliable and more definitive. Depigmentation - the loss of the darker areola color which it is supposed to have as compared to the surrounding skin - is always a risk with almost any procedure. But depigmentation is a well-known side effect of lasers. Lasers used on or near the pigmented areola can result in permanent, spotty depigmentation - very undesirable!

Pale, Depigmented Scars in the Areola: These can occur from previous trauma, procedures, surgery or lasers. The depigmented scar in the areola is unfortunately a very common occurrence in women who have had breast augmentation with implants placed via the areola incision approach. The best option is usually cosmetic tattooing.

Extra Nipple/Areola Complexes: Some patients have what might appear to be small moles on the chest or abdomen - but these may actually be extra nipple/areola complexes! These are also known as "accessory" or "supernumerary" nipples. Small, extra complexes can occur anywhere along the so-called "milk-line" which extends from the armpit through the center of the breast and down to the groin crease. A bump or lump underneath could also represent a small amount of breast tissue as well. It is generally agreed upon that it is important that these extra collections of breast related tissues be removed because of risks for malignant changes. Simple excision of these extra nipples is usually all that is required.

Post-Mastectomy Nipple/Areola Reconstruction: This is somewhat beyond the scope of this article, but certainly there are cosmetic issues involved in this very important aspect of breast reconstruction following any breast cancer treatment involving a mastectomy. Typically, nipple/areola reconstruction is not definitively planned and performed until all other aspects of the reconstruction of the breast are deemed complete and stable. Combinations of some of the techniques as described above - such as skin grafting, minor surgical procedures and tattooing - are all commonly employed.

The Problem With Cosmetic Tattooing


Scarring or keloids will often form with facial or make up tattooing. Many people are prone to keloids but all tattooing will cause some small level of scarring. The biggest problem this causes for facial tattooing is that when this scar tissue rises or puffs up around the eyes or lips the area becomes red and itchy. Dog Tear Stains Not Just Cosmetic


Dog tear stains are a cosmetic problem, caused by an overflow of tears onto the cheeks, which is most obvious on dogs with white and other light color coats. Tear accumulation on facial hair can lead to matting of the hair, skin irritation and possibly infection. Hair can act like a wick, drawing the tears out of the eye. This is correctable by removing the hair, keeping the area clean and removing accumulated material or crusts. It may take a while for your pet to adapt to this water so start with a little less and gradually increase the amount of vinegar.

An option that can be discussed with your vet is a common eye drop called napthazoline, which dilates the tear ducts so more tears flow where they are supposed to instead of over the lids and onto the face.

Another possibility is to put your dog on a very low dose of antibiotics which will eliminate bacterial overgrowth. However, this should be a last resort, not used for continued treatment, and not be considered for puppies without their adult teeth.

Dog tear staining is often more complex than simple answers offer.

Veterinary consultation is appropriate in determining the source of a dog tear staining problem. Have your vet give the dog a complete exam to rule out any serious eye conditions before trying anything.

Home remedies do exist, using mixtures of milk of magnesia, corn starch, bleach, hydrogen peroxide, boric acid and lemon juice. However, harsh ingredients can irritate and make tearing worse. A stain remover should be free of alcohol or bleach. If such ingredients aren't mixed properly, using precisely the right strength, and applied safely, you could potentially harm your dog.

No solution should be allowed to wick through the facial hair or splash into the pet's eye, or into the eye area. For this reason, caution suggests you look to over-the-counter products, and refrain from using these home formulas.

Use a quality over-the-counter dog tear stain remover to safely, effectively and gently minimize the condition for a "satisfactory enough" rather than complete elimination result. Unless your pet is a show dog, this is not really a serious issue. A protective sterile ophthalmic ointment under the eyes, when applying stain remover, is an excellent idea. It will keep the eyes comfortable because the filmy barrier prevents irritation.

Skin Lightening Options Threatened: Is Hydroquinone Really Too Dangerous for Consumer Use


First, the Food and Drug Administration restricted use of mercury as a lightening agent because of its caustic heath influences. Now, hydroquinone could face similar cosmetic prohibitions. But is the FDA's recent finding of hydroquinone as generally unsafe and not effective justified? Dermatologists and researchers have posted passionate views about the need to keep hydroquinone on the market as a skin lightener or as a minimum, allow it to remain available via prescription.

While such views abound, the question remains: Has the United States just lagged behind an international trend of banning potentially dangerous cosmetics and is now starting to catch up? Or, is hydroquinone still a safe skin lightening agent when used properly?

For years, consumers have used cosmetics containing hydroquinone for issues like melasma, also known as the "mask of pregnancy", age spots and acne scarring. As a skin lightener, hydroquinone hampers the production of the production of melanin, the pigment that gives the skin its color.

A recent medical review of hydroquinone published in the Journal of the European Academy of Dermatology and Venereology, concluded that doctors should recommend hydroquinone cautiously because of its potential, yet undocumented, ability to cause DNA damage and cancer in humans. However, such findings have not swayed medical professionals' confidence in the safety and effectiveness of hydroquinone.

In an interview with Skin and Allergy News, Dr. Valerie Callender, a dermatologist in private practice in Mitchellville, Maryland, said that it would be "ridiculous" to suggest that animal data could be used to raise concerns about human carcinogenicity. Yet, animal testing is generally the first step in determining if a new drug is safe for human consumption in clinical trials.

Even dermatologist Susan Taylor, M.D., assistant professor of dermatology at Columbia University, New York finds the link between hydroquinone and cancer dubious. Dr. Taylor reported in Dermatology Times, "If you give a rat enough of anything orally it may well develop cancer."

Besides cancer, ochronosis has surfaced as another side effect of hydroquinone usage. Ochronosis describes the darkened skin than can results from use of skin creams containing hydroquinone. Ochronosis has been largely documented in countries like Senegal, South Africa, Saudi Arabia and Nigeria where as much as 25% of the adult population may use skin lightening agents.

Reports in medical periodicals like the Journal of Dermatological Treatment and the American Journal of Clinical Dermatology link the preponderance of ochronosis to application of excessive amounts of hydroquinone and combining hydroquinone with chemical agents like mercury, phenols and potent acids.

Regardless of the battles over the relevance of medical studies on hydroquinone, the FDA faces heated arguments to banning hydroquinone usage. Medical associations like the American Academy of Dermatology asked its members to contest the proposed ban on hydroquinone by submitting their personal accounts of their experience with using hydroquinone on patients to the FDA for consideration.

While the FDA deliberates over the safety of hydroquinone, consumers still have multiple skin lightening options. Since hydroquinone was first classified as generally safe and effective in 1982, new chemicals and botanical ingredients have entered onto the scene as alternative skin lightening agents.

Certain cosmetic products contain hypoallergenic ingredients which cause fewer allergic reactions than ordinary cosmetic products. Although the chance of developing an allergic reaction to one of these substances is relatively small, they are not perfect and do cause such reactions in some of the people who use them.

Is a HIV Disaster Looming For India?


HIV has reached epidemic proportions in India and it is predicted that this nation will be next in line after sub-Saharan Africa to be devastated by the virus. HIV infection in India is currently concentrated among poor, marginalized groups, including commercial sex workers, truck drivers, and migrant labourers but if effective prevention efforts are not implemented immediately, India potentially could have 37 million people infected with HIV by the year 2005. To put this another way, this figure is roughly equal to the total number of HIV infections in the world today. Unfortunately, this rising HIV rate is also likely to fuel India's epidemic of tuberculosis as this pathology is the most common opportunistic infection in Indian patients with AIDS. Evidence from successful AIDS control projects around the world shows that targeted intervention programmes by peer educators among the groups most vulnerable to HIV are the most effective way to contain the rapid spread of HIV infection. Other interventions, such as Brazil's AIDS treatment program with antiretroviral drugs have also been successful. In 1997, the Brazilian Government started producing generic AIDS medicines and distributing them to HIV patients, free of charge. Although controversial at the time, the decision stands as a turning point in the battle against HIV and Latin America's most populous nation has become a showcase in the fight against the disease. Two decades ago, when the first cases of AIDS emerged in Brazil, health experts forecast that by now, the human immunodeficiency virus would afflict at least 1.2 million Brazilians. Instead, infection rates have returned to 1995 levels. Over the past five years, the number of AIDS-related deaths has plummeted in Rio de Janeiro and Sao Paulo, the regions most deeply affected. Several other developing countries are recognised for their successful public health response to HIV/AIDS. Thailand's ministry of health established a policy that all pregnant women should be provided with voluntary counselling and testing for HIV, offered zidovudine; and infants born to HIV-infected mothers are given zidovudine. In contrast, the South African government determined until recently that antiretroviral therapy for perinatal HIV prevention was too expensive (a decision, which cost over 70 000 infant lives each year). Despite this, many people feel that the current trajectory of the HIV/AIDS epidemic in India is unlikely to change short term and the nation has only a small but rapidly closing window of opportunity for stemming the epidemic. India's socio-economic status, cultural myths on sex and sexuality and a huge population of marginalised people make it extremely vulnerable to the HIV/AIDS epidemic. If they fail, the AIDS situation in India could become like that in many of the worst affected African countries.
